HOW DO I MAKE AN APPOINTMENT?
- Please call our office at 972-424-5437 and make an appointment.
WHAT DO I BRING TO MY FIRST VISIT?
- First, please plan to arrive earlier than your appointment time.
- Please bring your child’s insurance card and immunization record (if you have one).
- Please make sure you have changed Primary Care Physician (if your plan requires it) to DOLORES IFEZUE, MD (if you need help; give us a call).
- You will need to fill out some forms when you arrive. You can print these forms and fill them out before you arrive.
- Just click on the Patient Forms.
HOW WOULD I TRANSFER MY CHILDS MEDICAL RECORD TO YOU?
- Please go to Patient Forms section under the menu option Resources.
- Print and fill out the appropriate form and send to your previous doctor.
- You may choose to do so when you come to our office.
